Additional Terms for this Account
Interest will be compounded monthly. Interest will be credited monthly. Interest begins to accrue no later than the business day that we receive credit for the deposit of non-cash items (for example, checks). We use the daily balance method to calculate the interest on your account. The daily balance method applies a daily periodic rate to the principal in the account each day.

How Transactions (checks and debit card) on this Account are Limited
Purchases by check are limited only to the funds in the HSA. At point-of-sale merchants accepting debit cards, you may make no more than five (5) purchases each day. Maximum daily limit on the debit card is $5,000 if the Merchant is specifically categorized as a Qualified Medical Expense (QME) Merchant (ex. Hospitals, Dentists, Ambulance, etc.) and is $3,500 if the Merchant is a Non-QME Merchant. You may not make cash deposits from ATM terminals, but you can use ATMs for withdrawals with your PIN (there is a $2.00 fee) up to $300 per day. Other point-of-sale terminals or terminal operators may have other limits on the amount and frequency of purchases and may impose other fees. Checks and the debit card should be used for "normal distributions" only. Submit withdrawal forms to HSA Bank for death, disability, and prohibited distributions. Submit the Excess Contribution Form for excess contribution removal distributions.
